In the event on 1/12, there was no attendance recorded on the A.S attendance verification form. I see on the PEEF that the event did actually occur. A.S attendance verification is not optional; it is mandatory, why wasn't the form used? You may appeal by Friday, 4/11 with a thorough explanation and action plan for improvement.
